چکیده

Objective-To assess the effect ofa programme of postoperative community surveillance on the rate of detection ofwound complications after operation for inguinal hernia. Design-Prospective audit of wound complications including complications recorded in case notes and those discovered by community surveillance. Setting-Academic surgical unit of three consultant surgeons. Patients-510 patients undergoing elective inguinal hernia repair between June 1985 and August 1989. Results-The wound infection rate recorded in the hospital notes was 3% compared with 9% when additional information was obtained from community surveillance. Wound complications were detected in 143 (28%) patients by community surveiliance compared with a complication rate of7% in the case records for the same patients. Conclusions-Wound complications are common after clean surgery in patients discharged home early. Complication rates are a reflection not only of the standards of surgical practice but also the rigour with which they are sought. Before national comparative audit data are published the method of collection must be standardised. For short stay surgery this should include meaningful community surveillance.

A request for hospice admission from hospital to withdraw ventilation.

A request to admit a hospital inpatient with motor neurone disease to the hospice generated unusual unease. Significantly, withdrawal of ventilation had already been planned. The presumption that ventilation would be withdrawn after transfer presented a dilemma. Should the hospice accept the admission? If so, should the hospice staff stop the ventilation, and then when and how? Debate centred o...

A retrospective review of specialist palliative care involvement in motor neurone disease.

The provision of specialist palliative care to Irish patients suffering from motor neurone disease has not been described in the literature. The purpose of this study was to characterize the care provided at a Dublin hospice. Consecutive referrals between 1st January 1999 and 31st December 2008 (n=72) were reviewed. At the time of data collection, 61 (84.7%) were deceased, 9 (12.5%) were alive ...

C Cognition and behaviour in mot or neurone disease

Current Opinion in Neurology 2010, 23:638–642 Purpose of review Motor neurone disease has traditionally been considered a pure motor syndrome which spares aspects of cognition and behaviour, although in recent years it has been suggested that up to 50% of patients with motor neurone disease may develop frontal dysfunction which, in some cases, is severe enough to reach criteria for frontotempor...

The central nervous system in motor neurone disease.

Forty-five necropsied cases with primary degeneration of lower motor neurones are described and discussed. Of these, 36 are considered to be `typical' cases of motor neurone disease, eight of which showed no upper motor neurone lesions. The relation of the nine `atypical' cases to the remainder is discussed. It is concluded that motor neurone disease constitutes an ill-defined band in a broad s...

Motor neurone disease and exposure to lead.

Disease of the lower motor neurone is a recognized hazard of lead toxicity, but the importance of contact with lead in the causation of motor neurone disease has not previously been ascertained. In 74 cases of motor neurone disease, 15% had a history of extensive exposure to lead, compared with 5·4% of a control group. The five year survival rate of these patients was 54%, compared with 16% in ...
